Palliative Support & In-Home Hospice
How we help those living with an illness:
- Help you develop and evolve your care plan
- Work with other health care providers
- Offer in-home volunteer support and respite from specially trained volunteers
- Help you navigate the health-care system and if necessary assist with referrals to other supports
- 1:1 counselling and/or group support
- Support for those difficult family discussions (re: Advance Care Planning, Levels of Care, palliation)
- Support and programs for your caregivers and you family
- Participation in our weekly Day Program
- Participation in complementary therapies and wellness activities (Reiki, Yoga, Meditation, online activities, etc.)
Caregiver Support
Support for those caring for a loved either one at home or from afar:
- An individualized care plan
- Respite support in the home
- 1:1 counselling and/or group support
- Connection and referral to other community caregiver supports
- Participation in Wellness activities (Reiki, Yoga, Meditation, online activities)
- Family support
- Follow up aftercare and Bereavement support
Grief and Bereavement Support
Support for individuals grieving a loss or an impending loss:
- An individualized assessment and care plan development
- 1:1 Counselling and/or participation in a support group
- Participation in Peer drop-in groups
- Participation in Wellness activities Reiki, Yoga, Meditation, online activities)
- Participation in Memorial ceremonies and activities
Family and Child Support
Support for those caring for or living with a loved one who has a life-limiting illness:
- Facilitation of difficult discussions with the family and caregivers
- Children’s support, individually or in a group
- Children’s HUG’s Program (Helping Understand Grief)
- Participation in Wellness Activities, Community Evenings and Group activities
