HOSPICE CARE FOR OUR COMMUNITY.
In early Spring of 2021, Hospice Vaughan opened the first 12 bed palliative care hospice in Vaughan, ON. Our facility offers 24 hour/7 day a week compassionate and comprehensive palliative care for individuals facing end-of-life realities and their families. Each suite has large picture windows facing into the gardens, private bathroom, refrigerator and television.
Our suites are designed to embody a “home-like” atmosphere so that families and friends feel comfortable to visit and be with their loved ones throughout the day and night. Caregivers and family members will also have the opportunity to receive support through this journey.
All referrals to our Residence must be made through your family physician or health care provider.

We offer a full range of customized and group services to support individuals affected by life-limiting illness, their caregivers and their families. All services are offered free of charge to the community from our team of doctors, medical professionals, knowledgeable staff and specially-trained volunteers. We welcome individuals of all beliefs and customs into Hospice Vaughan and promise to do the most we can to support your spiritual needs.

WHAT YOU NEED TO KNOW…
REFERRAL REQUIREMENTS
- A Palliative Care Common Referral Form completed by a palliative care physician or a family physician providing palliative care to the patient.
- Patient prognosis of fewer than 90 days. A prognosis of 3-6 months will be added to the deferred waitlist.
- Documented consent from the patient or Power of Attorney/substitute decision maker.
- Name and contact information of the contact person for referral follow up purposes.
REFERRAL PROCESS
- Submit the completed Palliative Care Common Referral Form and referral documents by fax to Health Partner Gateway at 905-952-2404 AND to Hospice Vaughan at 289-256-2084.
