From diagnosis forward, the journey can be confusing, long, and anxiety-ridden for an individual and their family. At Hospice Vaughan, we provide a continuum of community services to support and embrace the whole person and their family and caregivers during that journey.
At Hospice Vaughan, we help with accessing health and social services, providing information, advocacy, and support from professional staff and trained volunteers. With support, each individual creates a personalized care plan that evolves with their changing needs. This care plan may include services such as psychosocial and emotional support, in-home social visits, and the opportunity to participate in educational workshops and wellness activities such as yoga, meditation, and reiki.
Providing grief and bereavement support is another way we support our community. Like our other services, each individual’s needs are met with personalized care and support through services such as individual counselling, support groups, and memorial opportunities.
